Having access to online services allows you to:
- manage your appointments - book, check or cancel appointments with a GP, nurse, or other healthcare professional
- order repeat prescriptions
- view your medical records including test results, hospital letters and more
To access online services you need to be registered with one of the following:
NHS App / NHS Account
You are able to register yourself on the NHS app. Please visit the NHS App website for more information.
SystmOnline / Airmid app
How to sign up for SystmOnline
- Print and complete the registration form below (online services registration forms are also available from reception)

- Return the completed form to reception. We will then check you are who you say you are. We will do this in one of three ways:
- photo ID and proof of address, for example, a passport or driving licence and a bank statement or council tax statement
- if you do not have any ID and are well known to us, a member of staff may be able to confirm your identity
- if you do not have any ID and are not well known to us, we may ask you questions about the information in your GP record to confirm the record is really yours.
- We will then give you a letter with your unique username and password. It will also tell you about the website where you can login and start using online services.

Proxy Access
A GP surgery can give proxy access to someone so they can help another person manage their GP health and care.
With proxy access someone can:
- order repeat prescriptions
- book appointments
- view medical records - such as test results, vaccination history, hospital letters etc.
Reasons to give someone proxy access may include:
- having a physical or mental health condition
- technical barriers, for example not having access to a computer or smartphone
- language barriers
- practical reasons, for example working unsocial hours
- age, for example a parent may want proxy access to manage their childs appointments or prescriptions

Once this is set up, if both the patient and the proxy are registered at the same surgery, you can access these services through the NHS App.
Proxy access can still be granted when the patient and proxy are registered at different surgeries, however, you will be unable to access these services via the NHS App and will need to use another online service provider.

This service is designed and run by the NHS, so your personal information is safe.
