Contact us online - Patient Triage

We are pleased to announce we have launched a new online consultation tool. Please read the following information to better understand how it works. We ask that patients use this responsibly and only submit routine requests, this service is not for urgent matters.

Patient triage can be used for the following:

  1. To seek self-help information from the NHS. Services include self-help information, advice or help from local services which may be appropriate for your condition, such as self-referral services or help from your local pharmacy.
  2. To seek medical advice online from Woodbrook Medical Centre for your condition.
  3. To seek general or administrative advice from Woodbrook Medical Centre. (E.g. for fit-notes, referrals, repeat prescriptions etc.)

Patient triage allows you to submit your query directly to the practice online; we ask that you provide as much detail as you can to our team. We will aim to view and respond to your query within 4-5 working days. This timeframe may vary depending on the number of queries we receive.

All medical queries received are booked into a clinical member of staffs clinic to allow them time to review and respond to your message

We will try to respond using your preferred method but we may feel that we can better meet your needs through an alternative contact method.

Please do not use this online patient triage system for urgent or emergency requests. For anything urgent you will need to call the surgery directly.

We will be monitoring this system closely and working to respond to patients as soon as possible, but as we cannot predict the volume of requests we are going to receive, we cannot guarantee we will see your request on the same day. We are stating a 4-5 working day turn around however, this may vary depending on the volume and type of requests we receive.

Our routine prescription request form will remain on the webiste, you can continue to use this as normal, it will remain accessible 24/7. The new online consultation tool does allow you to request repeat prescriptions but will not be accessible over weekends and bank holidays. The form is linked below:

The submitted forms will only be read during office hours. The service will not be available on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 18:30 – 08:00. Please contact 111 during out of hours or 999 if this is an emergency.
