Please read the following information to better understand how it works. We ask that patients use this responsibly and only submit routine requests, this service is not for urgent matters.
Patient triage can be used for the following:
To seek self-help information from the NHS. Services include self-help information, advice or help from local services which may be appropriate for your condition, such as self-referral services or help from your local pharmacy.
To submit general or administrative requests to Woodbrook Medical Centre, these can include:
- Requests for a continuation of a fit note (sick note)
- Requesting an update regarding a referral
- Requesting a letter from your GP
- Requesting copies of test results
- To cancel an upcoming appointment
Appointments can also be cancelled by logging into your SystmOnline account, the Airmid or NHS app and by calling the surgery. If you need to cancel an appointment with short notice, doing it online, in the app or by calling the surgery are the preferred methods to ensure the appointment can be released with enough time to allow it to be used by another patient
To submit medical requests regarding new or ongoing problems

Patient triage allows you to submit your query directly to the practice online; we ask that you provide as much detail as you can to our team. We will aim to view and respond to your query within 4-5 working days. This timeframe may vary depending on the number of queries we receive.
We will try to respond using your preferred method but we may feel that we can better meet your needs through an alternative contact method.
Please do not use this online patient triage system for urgent or emergency requests. For anything urgent you will need to call the surgery directly.
This service is monitored daily, however, if a response is required this can take up to 4-5 working days, depending on the volume of queries received.
The submitted forms will only be read during office hours. The service will not be available on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday to Friday between the hours of 6:30pm to 8:00am. Please contact 111 during out of hours or 999 if this is an emergency.
